New Patient Information

 

New Patients to bring for first visit:

• All Insurance Cards
• If patient has no insurance, we will need a photo ID
• List of all medications you are taking
• List of any allergies

*We ask that patients arrive 5-10 minutes before appointment.

**All minors must be accompanied by a parent or guardian on the first visit.

 

New Patients with the following insurances will need to be referred by their Primary Care Physician:

• Medicaid
• Wellcare
• Amerigroup
• Peach State
• South East Community Care
• Tricare Standard
